How to Download Forms

1. Log in to bankVOD

2. Navigate to Download Forms from main menu

3. View list of available forms by category

Step 2: Select Form Category

1. Locate the category you need

2. Review available forms in that category

3. Identify the specific form required

Step 3: Download the Form

1. Click on form name or download link

2. PDF opens in new window/tab

3. Save to your computer

4. Or print directly if needed

Step 4: Complete the Form

For Interactive PDFs:

• Type directly into form fields

• Save completed form

• Print or upload as needed

For Non-Interactive PDFs:

• Print blank form

• Complete by hand in ink

Form-Specific Guidelines

Authorization Forms:

Critical Requirements:

• Borrower signature required

• Date must be current

• Legal name must match account records

• Include all account holders if joint account

• Clear and legible

Bank-Specific Forms:

When Required:

• Some banks require their own forms
